Infertility Treatment Q & A

What is the first step in infertility treatment?

Your infertility treatment begins with a consultation at Noble Fertility Center, during which both you and your partner (if applicable) have a thorough evaluation and exam to identify any fertility issues. Dr. Chang reviews any previous tests and treatments to help create the best individual care plan for your future treatments.

Diagnostic testing at Noble Fertility Center may include:

  • Baseline hormone tests
  • Day three hormone levels, including FSH/estradiol and/or AMH to check ovarian reserve
  • Day 21 progesterone to confirm the ovulatory status
  • Hysterosalpingogram to evaluate the fallopian tubes and uterine cavity
  • Sonohysterogram/hysteroscopy to evaluate uterus health
  • Semen analysis to rule out male factor infertility

If you have experienced recurrent miscarriage, defined as two or three consecutive pregnancy losses, Dr. Chang offers a full workup to identify and treat any possible underlying conditions causing your difficulties.

Women or couples may choose to use donor sperm, depending on their circumstances.

What does infertility treatment involve?

Noble Fertility Center customizes your infertility treatment according to many factors, including your age, medical history, pregnancy history, prior fertility treatments, and the suspected reason for your trouble conceiving.

Infertility treatment may include:

Ovulation induction

Medications like clomiphene citrate (Clomid® and Serophene®) or injectable medications like Follistim® or Gonal F® help your ovaries develop multiple, viable eggs. You then conceive naturally or through intrauterine insemination.

Intrauterine insemination

During this procedure, Dr. Chang places your partner’s sperm (or a donor’s) directly into your uterus to await an egg to fertilize. That bypasses issues with cervical mucus or other problems preventing fertilization.

Reproductive surgeries

Dr. Chang can perform laparoscopy and hysteroscopy to correct issues like endometriosis, fibroids, polyps, or pelvic adhesions that interfere with your ability to get pregnant.

Gestational carrier

A gestational carrier is a woman who carries a baby for another couple. This carrier returns the baby to you, the genetic parent, after delivery.

In vitro fertilization (IVF)

IVF involves introducing your egg and your partner’s sperm (or donor sperm) in a lab to create a viable embryo. Dr. Chang places the best embryos into your uterus for hopeful implantation and a healthy pregnancy.

Couples undergoing IVF may choose preimplantation genetic screening and diagnosis (PGS/PGD) to assess the health of embryos prior to their transfer. The fertility technicians may also perform Embryoplasty™ to improve embryo integrity before transfer.

Noble Fertility Center also offers assisted hatching of viable embryos. The belief is that assisted hatching can help an embryo implant in the uterus, leading to higher pregnancy rates in some patients.

Egg freezing and embryo freezing

Noble Fertility Center provides egg freezing and embryo freezing (cryostorage) for long-term storage and future fertility treatments.

What is male infertility treatment?

Certain sperm deficiencies, such as sperm quantity, shape, and movement, can interfere with the fertilization of an egg. These deficiencies are known as “male factor infertility.” Dr. Chang can identify infertility due to the man and may offer intracytoplasmic sperm injection (ICSI) as a treatment.

During ICSI, Dr. Chang individually injects sperm into each mature egg, using a powerful microscope and specially designed tools to increase the chance of fertilization. He places any resulting embryos in the woman’s uterus in an in vitro process.
